Please note, we are ONLY discontinuing the CareConnections program. ClearSky Rehabilitation Hospital of Rosepine continues to fully operate in providing specialized rehabilitative care to those with disabling injuries or illnesses.

Dear ClearSky Care Connections Patients,

ClearSky Care Connections, LLC (d/b/a ClearSky Care Connections Louisiana) (“Care Connections”) has decided to voluntarily close operations and stop seeing patients.  You may check with your insurance provider for other in-network providers.

Your medical records will be kept private in accordance with applicable State and Federal law. If you need copies of your medical records, please contact us at the following email address mmedina@clearskyhealth.com and submit the medical records authorization form. Otherwise, your patient records will be securely stored at the below location for the time period and to the extent required by law.
