Managing a Chronic Illness

Living with a chronic illness, such as Parkinson’s disease or multiple sclerosis, can be challenging. At ClearSky Rehabilitation Hospital of Elwood-St. Joseph, we help you better manage and control symptoms of your illness.

Our physician-led, interdisciplinary healthcare team customizes a treatment plan with you and your family based upon your individual goals, motivations, and abilities. We question, challenge, and strive to get better ongoing results for you during the course of your care.

Depending upon your needs, we can help you address a number of issues, including:

  • Medication management
  • Muscle tone/tremor management
  • Walking, balance, and posture difficulties
  • Fatigue and endurance
  • Use of adaptive equipment
  • Swallowing issues
  • Impaired memory, problem-solving, and behavior
  • Communication skills
  • Self-care skills, such as feeding and dressing
  • Bowel and bladder training
  • Voice and speech impairments
  • Safety awareness

Upon discharge, we remain a partner in your healthcare. We understand that your best opportunity for recovery is within a few months following your medical event. We want to help you find ways to capitalize on that time.

We help arrange further resources as needed, such as follow-up physician appointments, outpatient therapy, or home health. We supply you and your family with educational information on continued exercises, behavior modifications, home modifications, and assistive devices, if necessary.
