ClearSky Health Acquisition, LLC, is required to provide you with Form 1095-C, which reports information about the health coverage we offered to you during the 2025 calendar year. This form may be important for your tax filing purposes.

You do not need this form to file your taxes unless you purchased health insurance through the Health Insurance Marketplace and plan to claim the premium tax credit.
